This app was a design proposal created to showcase the design improvements in the existing caseworker app.
As the UX designer on the team, to reimagine the caseworker app I had to understand how a caseworker needed to be fully prepared for their client visits before designing and delivering the final wireframes.

This app would help the caseworker to prepare for client visits and then deliver quality care in the convenience of their home or wherever their family may be. They must have the ability to search for a registered client in a case management system and view all the information they need. This could be essentially achieved with the app and also help the caseworker to take immediate and appropriate action at the right time.
Analysis of the current application helped us to understand the caseworker's current responsibilities. With this, we sketched out the user persona, enhanced existing workflows and identified key use cases and scenarios.
The ideation process and sketching the persona identify the three screens that would enhance the current functionalities.
